Social care packages
Our children and young people frequently require care packages supporting a number of conditions involving learning disabilities, physical disabilities and medical health.
We work closely with social services and case managers to provide home help and short breaks for families. All our carers will have the relevant compliance, experience and background before placement in a person’s home ensuring continuity and reliability to build confidence and trust.
Focused Healthcare is aware of integrated service delivery between education, social services, voluntary sector and health sector to meet the needs of children. We support bespoke independent living care plans for children and young people in the community providing personal care assisting in activities of daily living and home help provision.
Each package of care is allocated its own clinical nurse manager who will prepare risk assessments, care plans, work in partnership with the family and support the care workers allocated to care for the child and young person.
Policies and procedures are in place to ensure children are safe and protected in their own homes. We look after children and young person with:
- Down syndrome
- Cerebral palsy
- Attention deficit disorder
- Autism disorder
- Social anxiety
and other complex social care conditions.
