Implementation of care plans
Focused Healthcare’s experience in providing services to meet the individual requirements of children and young people with complex care needs means that we are ideally placed to understand and meet the requirements of service commissioners.
Our role includes:
(I) Coordinating care packing implementation
Preparing an appropriate care plan, recruiting, maintaining and training carers, providing practice facilitators to support the parents and care team.
(II) Acting as family advocate
Discussing care plans and family concerns with multi professionals including local authorities, leads for continuing care and commissioners. Informing the family of what is happening at all stages of care plan implementation and acting as a support and resource for signposting.
(III) Ensuring every child matters
Our Clinical Nurse Managers are committed to the philosophy of Every child matters and the five Outcomes for children: Be healthy, Stay safe, Enjoy and achieve, Make a positive contribution, Achieve economic well being.
(IV) Management of risk
Our Clinical Nurse Managers are qualified Paediatric Nurses trained fully in management of risk and putting in place controls and checks within all our assessments.
The process of implementing a care plan
After receiving an enquiry from service commissioners we conduct a full initial and risk assessment of the child needs including environment. This includes a full review of case notes, meeting with the child or young person and family, visits to hospital ward, professional meetings and a home visit with a family member present.
The Clinical Nurse Manager will face many dilemmas and challenges in working with varied and complex cases and will apply a code of ethics that ensures the interests of the child takes preference at all times.
